Simple Steps Before Your Appointment

The Patient Center helps you plan your visit, complete forms efficiently, and understand what to bring. If you have questions at any point, contact the office and the team will help you choose the right next step.

New Patient Registration

Completing registration forms before arrival helps the visit start on time and keeps the focus on your care.

Bring the following to your appointment:

  • Photo ID

  • Vision and medical insurance cards, if applicable

  • Current glasses and contact lenses, if you wear them

  • A list of medications and any known allergies

  • Any relevant eye records or surgical history, if available

If you prefer to complete forms in the office, arrive a few minutes early so the team can assist.

Insurance and Billing Basics

Insurance can feel confusing because eye care includes both routine vision services and medical evaluation when symptoms or disease monitoring are involved.

Coverage depends on your plan and the services provided. The team can verify benefits and explain how your plan applies before your visit whenever possible.

If you have questions about copays, deductibles, or what applies to eyewear or contact lenses, ask before the appointment or at check-in. A clear explanation reduces surprises later.

What to Expect During Your Appointment

A typical visit includes check-in, a review of your vision and health history, and testing based on your needs. The eye doctor explains findings and discusses next steps in plain language.

If the optometrist needs additional detail, diagnostic testing may support evaluation and long-term monitoring.
